Anyone who cannot afford the player fee will not be refused participation. Parents must give notice to the Recreation Department if any financial problem exists. There will be a meeting with the Youth Baseball Board to decide payment arrangements.

I agree to return, upon request, the uniform and other equipment issued to players in as good condition as when it is received, except for normal wear and tear.

I also grant permission to managing personnel or other Recreation Department representatives to obtain and authorize medical care should the participant become ill or injured while participating in activities away from home, or at times when neither parent is available to grant authorization for emergency treatment.

I will furnish a birth certificate. I will be responsible to assure that my child is physically able to participate in the program prior to enrolling.

I, parent or guardian of the above-named candidate for participation in the baseball program, hereby give approval in any and all activities during the current season. I assume all risks and hazards incidental to and from the activities and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local league organization, the organizers, sponsors, schools, supervisors, participants, and persons transporting the player to and from activities for any claim arising out of an injury to the participant.
